Delhi Man Recovers After Six Surgeries and Rare Skull Bone TB

A 53-year-old man from Delhi has made a remarkable recovery after spending nearly two and a half years undergoing intensive medical treatment, multiple hospital admissions and six major surgeries. His medical journey began with a sudden and life-threatening bleeding episode in the brain that left him in a semi-comatose condition. What followed was a complicated chain of medical problems that required repeated surgical intervention, prolonged intensive care, treatment for infections and extensive rehabilitation. During his recovery, doctors discovered an extremely uncommon form of tuberculosis affecting the bone of the skull. The case became particularly complex because the patient developed several additional complications involving the brain, spine, nervous system and vision. Despite these setbacks, a team of specialists continued to modify his treatment as new problems emerged.

A Sudden Brain Haemorrhage Changed Everything

The man's medical journey began when he suddenly lost consciousness because of a spontaneous haemorrhage inside the brain. The bleeding caused severe swelling, creating dangerous pressure within the skull. His condition was particularly difficult to manage because he already had factors that increased the risk of bleeding. Doctors said he had a bleeding disorder, a low platelet count and was taking Ecosprin for a heart-related condition.

The combination made emergency treatment especially challenging.

Doctors had to act quickly to prevent further damage to the brain. They performed an emergency decompressive operation to remove the blood clot and reduce pressure caused by the swelling. Because the brain had swollen significantly, part of the skull was temporarily left open. This approach was intended to provide additional space for the swollen brain and reduce the possibility of permanent neurological damage. Following the operation, the patient needed prolonged breathing support and underwent a tracheostomy. He remained in intensive care for almost a month before gradually regaining consciousness. After his condition stabilised, he was discharged, but his medical battle was far from over.

Seizures Led to Another Major Operation

After the initial recovery, the patient developed seizures. Doctors subsequently had to perform another operation to reconstruct the part of the skull that had previously been left open. For this procedure, the patient's own skull bone was used. The bone had been preserved in a bone bank after the earlier surgery. However, another serious complication appeared soon after the reconstruction. A new blood clot developed beneath the replaced section of the skull. Doctors were forced to perform another urgent brain operation to address the clot and protect the patient from further complications. At this stage, the patient's treatment had already become a long-term medical challenge involving repeated neurological procedures and close monitoring.

Rare Tuberculosis Was Discovered in the Skull Bone

Several months later, doctors noticed discharge coming from the surgical wound. Further medical investigations revealed that the bone used in the skull reconstruction had developed tuberculosis. Tuberculosis generally affects the lungs, but it can also occur in other parts of the body. In this case, the infection involved the skull bone, making the condition exceptionally uncommon. Doctors removed the infected bone and began prolonged anti-tuberculosis treatment. The medical situation became even more complicated when investigations indicated that tuberculosis had also affected the patient's brain and spine. This meant doctors had to treat not only the original neurological problems but also a serious infectious disease involving multiple areas of the central nervous system. The combination of repeated surgery, infection and neurological complications required continuous reassessment of his treatment plan.

Multiple Hospital Admissions Added to the Challenge

The patient's recovery continued to be interrupted by repeated episodes of neurological weakness and recurrent infections. He required several more hospitalisations as doctors worked to control infections and manage complications associated with his previous procedures. During this prolonged period, he also suffered a fracture of the thigh bone after a fall, adding another major medical problem to an already complicated recovery. Instead of treating the problems separately, specialists eventually carried out a multidisciplinary procedure in which the fracture was repaired while the skull was reconstructed. For the skull reconstruction, doctors used a custom-designed PEEK cranial implant. PEEK, or Polyether Ether Ketone, is a high-performance material that can be used in specialised medical implants. In this case, the implant was designed according to the patient's individual anatomy using detailed imaging.

Treatment Also Affected His Vision

Another major complication emerged toward the later stages of treatment. The patient developed optic neuritis, an inflammatory condition affecting the optic nerve, which resulted in partial loss of vision. Doctors detected the problem at an early stage and changed the treatment regimen. Rehabilitation was then continued while the medical team closely monitored his vision and overall neurological condition. The complication demonstrated the difficulty of maintaining a long-term treatment plan when a patient is dealing with several serious conditions at the same time. The doctors had to balance the need to control the infection with the possibility of treatment-related side effects.

Six Surgeries and Nine Hospitalisations

Over a period of approximately two and a half years, the patient underwent six surgeries and nine hospitalisations. His treatment involved specialists from a wide range of medical disciplines, including neurosurgery, orthopaedics, infectious diseases, critical care, cardiology, pulmonology, nephrology, urology, ophthalmology, endocrinology, gastroenterology and rehabilitation medicine. The involvement of so many departments reflected the complexity of his condition. Rather than dealing with a single illness, doctors were required to manage a sequence of interconnected problems that developed at different stages of his recovery. From the initial brain haemorrhage to skull reconstruction, infection, tuberculosis affecting the skull, brain and spine, a bone fracture and vision-related complications, each stage required a different medical approach.

Doctors Explain Why the Case Was Unusual

Dr Sonal Gupta, Principal Director and Head of Neuro and Spine Surgery at Fortis Hospital, Shalimar Bagh, said one of the biggest challenges was managing the initial emergency in a patient who was already at high risk of bleeding. She said the subsequent two-and-a-half-year period was equally challenging because of the complications that developed after the first operation. The doctors highlighted that tuberculosis involving a skull bone following brain surgery is extremely uncommon. They also noted that infections involving cranioplasty bone flaps occur in a small proportion of cases. According to Gupta, the overall infection rate for such bone flaps is around 5–6 per cent, but most infections are caused by bacteria. Tuberculosis affecting the bone flap is much rarer, with only a limited number of such cases described in medical literature.

A Long Road From Critical Illness to Independence

The case represents an unusually long medical journey in which the patient's condition repeatedly changed. His initial situation involved a serious brain bleed and extensive swelling. After emergency surgery and intensive care, he faced seizures and further neurological surgery. He then developed another blood clot, followed months later by infection at the surgical site. Investigations revealed the rare bone infection, followed by the discovery of tuberculosis involving the brain and spine. Repeated infections and neurological weakness resulted in further hospital stays. A fall then caused a thigh-bone fracture, requiring orthopaedic treatment. The skull was ultimately reconstructed with a personalised implant while doctors repaired the fracture. Even after the major procedures were completed, treatment-related complications remained a concern. The development of optic neuritis required doctors to modify his medication and continue rehabilitation.
